Membership Number

• The membership number is unique to each of your accounts. Kindly quote the membership number for all your transactions and correspondences with AHMTPL.

Membership Card

• “AHMTPL” means Amazon Health & Medical Tourism Pvt Ltd.
• Please carry your card during all your future visits to AHMTPL center and present it while billing.
• At the time of billing, please notify the attending personnel about your card to avail discounts purchases (products and / or services) made.
• In case you forget to carry your loyalty card during your visit, kindly notify the attending personnel before billing. Give your membership number or answer questions based on the details provided by you in your Customer Application Form to avail privileges.
• Please note that this procedure is part of our safety protocol that has been devised to safeguard your interests and to prevent possible misuse of your card.
• The card can be used at any authorized AHMTPL center across India.
• No cash refund will be entertained for purchases made.
• Membership validity is for the period of 1 year from the date of issue.
• The card is not transferable
• Wellness Card value – Rs. 800 / 20$
• Second year onwards on membership get 20% discount
• Permanent care will be issued on next year renewal

Card Lost//Stolen

• In case the card is either lost, misplaced by you or is stolen, immediately notify the issuing center of AHMTPL in writing to prevent any kind of misuse.

• The card can be used at any authorized AHMTPL center across India.
